Bedford, Building F, Gladstone, Missouri- Employees were exposed to overhead hazards while working and walking underneath elevated materials and equipment and were not protected from impact by the use of a hard hat.

Bedford, Building F, Gladstone, Missouri-Employees were exposed to fall hazards while on the ends of the pumpjack scaffold that was not provided with endrails.

Bedford, Building F, Gladstone, Missouri- Employees were exposed to a fall hazard while using a portable ladder to access the platform. The ladder being used did not extend at least 3 feet above the upper landing surface.
