29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:  a.  On or about October 17, 2023, at the above addressed jobsite, an employee performing residential roofing operations was required to work within close proximity to energized lines, thereby exposing the employee to electrical hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about October 17, 2023, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet five (5) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about October 17, 2023, at the above addressed jobsite, the portable ladder used to access and egress the roof's surface did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to fall hazards.
