29 CFR  1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:  a) Jobsite at 1818 Market Street, Philadelphia, PA - Sidewalk in front of building: On or about October 23, 2023, the employer did not provide a fit test to all employees required to wear respirators, exposing them to respiratory hazards. The employer required an employee to wear a respirator such as, but not limited to a 3M 6391 half facepiece respirator with 3M P100 filters while performing concrete cutting and chipping activities.
