On or about September 30, 2023 at the address of 46 East Main St. Shelby OH, 44875: The employer did not provide a training program for each employee potentially exposed to fall hazards while using a 32 feet fiberglass extension ladder to access the second level of a commercial roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.

On or about September 30, 2023 at the address of 46 East Main St. Shelby OH, 44875: The employer did not ensure employees were effectively protected from live conductors of an energized electrical service line by means of deenergization, insulation or proximity guarding during roofing work activities (installing EPDM and gutter system). Employees operating within 1foot of the 120V/240V energized electrical service line were exposed to electrical shock/electrocution.

On or about September 30, 2023 at the address of 46 East Main St. Shelby OH, 44875: The employer failed to report on employee hospitalization (admittance) following a workplace incident (fall from a ladder). The employer or employer representative failed to contact the Toledo Area Office or the 24-hour OSHA number to notify the agency of the employee hospitalization within 24 hours of admittance. The injured employee was admitted on September 30, 2023, for related in patient treatment. The employer did not report the incident or made an attempt to report the incident to the Toledo Area Office.
