29 CFR 1910.30(b)(1): The employer failed to train each employee in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment:  a) Suspension Span, South Channel Bridge, International Bridge Road, Massena, NY 13662, on or about 10/20/2023: The employer failed to adequately ensure that each employee was trained on and correctly understood the safe work procedures associated with completing the inspection activities on a Liftsafe Engineering & Service Group Maintenance Traveller that traversed the underside of the bridge.

29 CFR 1910.132(d)(1) The employer failed to assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Suspension Span, South Channel Bridge, International Bridge Road, Massena, NY 13662, on or about 10/20/2023: The employer did not adequately assess the workplace hazards associated with completing the inspection activities on a Liftsafe Engineering & Service Group Maintenance Traveller including hazards including, but not limited to, crush-by, struck-by, caught- in, pinch, fall, and high noise exposures during bridge inspection activities.
