29 CFR  1910.132(d)(1):  The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a)  At FM Meat Products LP, located at 19798 NE Hwy 315, Fort McCoy, FL: On or about 10/19/2023 employees were exposed to amputation hazards during hook exchange on the platform in the quartering room in that an adequate job hazard analysis was not conducted.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) At FM Meat Products LP, located at 19798 NE Hwy 315, Fort McCoy, FL in the Quartering Room: On or about 10/19/2023 employees were exposed to caught-in and caught-between hazards, in that the employer failed to place machine guarding on the descending section of an overhead, powered arm conveyor system at the platform where hook exchanges are conducted.
