29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equaled or exceeded an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a) A trim saw operator was exposed to noise levels at 328.3% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 328.3% is approximately 98.5 dBA.  Sampling was performed for 534 minutes during one shift.  Zero exposure was assumed for unsampled time.   (b) A trim saw operator/stacker was exposed to noise levels at 305.3% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 305.3% is approximately 98.0 dBA.  Sampling was performed for 527 minutes during one shift.  Zero exposure was assumed for unsampled time.   (c) A trim saw stacker was exposed to noise levels at 175.5% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 175.5% is approximately 94.0 dBA.  Sampling was performed for 454 minutes during one shift.  Zero exposure was assumed for unsampled time.   (d) A pallet dismantler operator was exposed to noise levels at 286.7% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 286.7% is approximately 97.5 dBA.  Sampling was performed for 459 minutes during one shift.  Zero exposure was assumed for unsampled time.   (e) A pallet dismantler operator was exposed to noise levels at 421.2% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 421.2% is approximately 100.3 dBA.  Sampling was performed for 457 minutes during one shift.  Zero exposure was assumed for unsampled time.
