29 CFR  1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it:   On or about October 24, 2023, during steel erection work on a new commercial building addition located at 9501 Allen Drive, Valleyview, OH 44125, the employer did not ensure employees working on a SkyJack mobile scaffold, SJ6826 RT, maintained feet on the floor and did not overload the guardrail system. Employees were observed standing on the midrail section of the guardrail system performing welding work.

29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:   On or about October 24, 2023, during steel erection work on a new commercial building addition located at 9501 Allen Drive, Valleyview, OH 44125, the employer did not ensure employees working on a SkyJack mobile scaffold, SJ6826 RT, worked from a scaffold platform at least 18 inches wide. Employees were observed standing on the midrail section of the guardrail system performing welding work.

29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  On or about October 24, 2023, during steel erection work on a new commercial building addition located at 9501 Allen Drive, Valley View, OH 44125, the employer did not ensure employees used a fall protection system/device while performing final detail work. Employees were exposed to fall hazards of approximately 30 feet onto a compacted gravel surface. The employer completed connecting work and no crane was in use at the time of the inspection.
