29 CFR  1926.454(b):  The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  a) On October 23, 2023, at the jobsite, where the employer did not provide training that prohibited employees from standing on the guardrails of the JLG Scissor lift.

29 CFR  1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  On or about October 23, 2023, at the jobsite, where the employer did not provide fall protection for employees installing a heating, ventilation, and air conditioning (HVAC) unit on the roof of a new building structure.
