29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about October 25, 2023, employees were performing roofing activities such as, but not limited to installing underlayment, shingles, and flashing at heights greater than 6-feet, where the following:  a) On Building 2512 - ladders to access the roof from the ground level side rails did not extend 3-feet above the roof surface.  b) On Building 2524 - ladders to access the roof from the ground level side rails did not extend 3-feet above the roof surface.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  On or about October 25, 2023, employees were performing roofing activities such as, but not limited to installing underlayment, shingles, and flashing at heights greater than 6-feet where the following:   a) On Building 2512 used a ladder to access the roof from the ground level where the ladder was placed at angles less than 75 degrees.  b) On Building 2524 used a ladder to access the roof from the ground level where the ladder was placed at angles less than 75 degrees.  c) On Building 2532 used a ladder to access the roof from the ground level where the ladder was placed at angles less than 75 degrees.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 25, 2023, employees were performing roofing activities such as, but not limited to installing underlayment, shingles, and flashing at heights greater than 6-feet, without fall protection on the following:  a) Building 2512;  b) Building 2524:  c) Building 2532.  Ochoa Andrade Construction Co Inc was previously cited for a repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1541910, citation number 1, item number 1 and was affirmed as a final order on January 18, 2022, with respect to a workplace located at 3201 Grant Rd, Norman, OK, 73071.
