29 CFR  1926.451(c)(1)(iii):Ties, guys, braces, or outriggers were not installed to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, such as a cantilevered work platform, was applied or was transmitted to the scaffold.   On or about October 24, 2023, employees were performing masonry activities on a commercial facility located at 4020 Brier Creek Parkway, Uniontown, Ohio 44685. The employees were working from a supported scaffold at a height of approximately 20 feet. The width of the base of the scaffold measured five feet. The platform height measured 20 feet and the employer had not secured the scaffold to the building to prevent tipping. The employees were exposed to the 20-foot fall to the ground.

The employees were working from a supported scaffold at a height of approximately 20 feet. Two separate scaffolding poles were found without mud sills or other adequate firm foundation. The employees were exposed to the 20-foot fall to the ground.

29 CFR  1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about October 24, 2023, employees were exposed to a fall of approximately 20 feet while performing masonry activities on a residential home located at 4020 Brier Creek Parkway, Uniontown, Ohio 44685.  The employees were on the upper level of a 20-foot supported scaffold. The employer did not provide a competent person to inspect the scaffold prior to start of work.

The employees were working from a supported scaffold at a height of approximately 20 feet. The employees were on the working level of the scaffolding without any form of fall protection. 29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces shall not be used as a means of access.  Employees were performing masonry activities on a commercial facility located at 4020 Brier Creek Parkway, Uniontown, Ohio 44685. The employees were working from a supported scaffold at a height of approximately 20 feet, the employer did not install a safe access point.
