Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:  On or about October 26, 2023, eight roofers employed by Carlos Hernandez LLC at the job site located at 1606 Rachel Elizabeth Lane in Wentzville Missouri were exposed to fall hazards. They were not utilizing fall protection such as a guardrail system, a safety net system, or a personal fall arrest system. These roofers were exposed to fall hazards from the roof at approximately 10 feet high from the ground.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper-level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about October 27, 2023, Carlos Hernandez LLC exposed 8 roofers to fall hazards from a roof approximately 10 feet high from the ground, wherein the ladder was not extended three feet beyond the landing surface or edge of the roof at 1606 Rachel Elizabeth Lane in Wentzville Missouri.
