29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  A): 1550 White Plains Road, Bronx - NY: An employee was exposed to fall hazards and did fall approximately 10-feet while working on fabricated framed scaffold. The exposed employee stepped off of the last step of the scaffold and fell off of the side approximately 10-feet to the ground below. The scaffold did not have a complete guardrail system or any other means of fall protection, on or about, October 19th, 2023.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  A): 1550 White Plains Road, Bronx - NY: The employer (Kodra Contracting Corp.) failed to notify OSHA of an employee accident that resulted in an employee in-patient hospitalization. The accident happened on October 19th, 2023, and was reported anonymously to the TTAO on October 24th, 2023.
