29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:   a) 3608 Kobuk St, Evans Colorado: On or about October 31, 2023, and at times prior, employees were exposed to fall hazards while placing plywood on the second story of a new residential home without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 3608 Kobuk St, Evans Colorado: On or about October 31, 2023, and at times prior, employees were exposed to fall hazards while accessing/egressing the roof. The ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.
