29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  Location: Jobsite 1 Cooper Way, Mattapoisett, MA  On November 1, 2023 , employees were exposed to falls from heights up to 25 feet to the ground while framing a house where fall protection training was not provided to recognize and minimize fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location: Jobsite 1 Cooper Way, Mattapoisett, MA  On November 1, 2023, employees were exposed to falls from heights up to 12 feet to the ground while accessing an upper landing surface where an extension ladder was used to access a residential roof and the ladder side rails did not extend at least 3 feet above the edge of the roof.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: Jobsite 1 Cooper Way, Mattapoisett, MA  On  November 1, 2023, workers were exposed to fall hazards of up to 25 feet to the ground below while framing a house where fall protection was not used.  JDS Framing Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1425977, citation number 1, item number 1, and was affirmed as a final order on January 6, 2020, with respect to a workplace located at Willis St. Apartments, New Bedford, MA.
