29 CFR  1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) 4542 Baldwin Ave, Montgomery, AL, Slitting Machine; On or about October 31, 2023 and times the employer exposed employees to fall hazards in that employees were required to perform work operations such as, but not limited to, replacing the bar on the tensioner, while at or near the edge of a scrap pit with a depth of around 30 feet.

29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:   a) 1936 Deer Street Co Rd 11, Ozark, AL, Equipment Building; On or about October 31, 2023 and times prior the employer failed to verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated, the person certifying that the evaluation had been performed, the date(s) of the hazard assessment, and which identified the document as a certification of hazard assessment for equipment such as, but not limited to, the slitting machine
