29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about 1 November 2023, employees of Wilson Arrasola dba Wilson Arrasola were exposed to fall hazards while working on the roof of a residential building located at 177 Bear Creek Drive in Wentzville, Missouri. The employees were removing roofing materials without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about 1 November 2023, employees of Wilson Arrasola dba Wilson Arrasola were exposed to fall hazards at 177 Bear Creek Drive in Wentzville, Missouri. The employees were installing roofing materials on the second-story of a residential building. The employees accessed the roof using a ladder that was not extended 3 feet beyond the upper landing surface.
