Sylvania OH 43560, did not ensure the side rails of one portable Werner brand  12-foot fiberglass multi-section extension ladder was extended at least three feet above the landing surface. Employees were exposed to falls from approximately 10 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential construction framing activities.
