29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  (a)	Caleta Construction LLC at 116 Cliffrose Way Glenwood Springs, CO 81601: On or about November 1, 2023, and at times prior, employees were exposed to fall hazards of approximately 10 feet  while conducting framing operations  without fall protection.

(a) Caleta Construction LLC at 116 Cliffrose Way Glenwood Springs, CO 81601:  On November 2, 2023, and at times prior, employees were exposed to struck by and caught in hazards while working  in a trench that was approximately 7 feet 6 inches in depth with spoil piles directly on the edge.

29 CFR 1926.651(k)(2):  Where the competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions had been taken to ensure their safety:  a) Caleta Construction LLC at 116 Cliffrose Way Glenwood Springs, CO 81601:  On November 1 , 2023, and at times prior, employees were allowed to enter the excavation without any of these necessary precautions in place.  The trench was measured to be approximately 7 feet 6 inches in depth in Type B soil.
