29 CFR  1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:   (a)	On or about November 6, 2023, the employer did not maintain workspace of at least 3 feet in front of General Electric Breaker Switch box 240-40-volt electrical panels located in the northeast corner of the shop, in that the panels were blocked by metal racks holding food products.  This condition exposed employees to electrical and mechanical hazards.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a) Super Bird Creations LLC,746 Kimball Avenue, Grand Junction, CO : On or about November 6, 2023, and at times prior, employees were exposed to struck by hazards while operating a Komatsu Forklift Truck (Model FG10-15, Serial Number 302285) without proper operator training and certifications.
