The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 2 for hazards associated with lack of fall protection training program.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about October 19, 2023, during a roofing project on a residential home located at 7113 Cedar St, Chagrin Falls, OH 44022, the employer did not ensure employees working on the roof were protected from a fall of approximately 25 feet by utilizing a personal fall arrest system or any other form of fall protection system.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about October 19, 2023, during a roofing project on a residential home located at 7113 Cedar St, Chagrin Falls, OH 44022, employee(s) conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about October 19, 2023, the employer did not report the in-patient hospitalization of an employee that suffered a broken left ankle and a broken right ankle as a result of a fall from approximately 25 feet within 24 hours. The employee was hospitalized for the incident and has not returned to work yet. OSHA was not notified until November 01, 2023.
