Location: 14 Sterling Street, Newton, MA; On or about 11/7/2023, employees were allowed to work from a pumpjack scaffolding at a height of 16-feet and the poles/legs of the pumpjack scaffolding were not secured to the structure by rigid triangular bracing at both top and bottom.

Location: 14 Sterling Street, Newton, MA; On or about 11/7/2023, workers were allowed to work inside an unprotected trenching excavation that was 7-feet in depth and were exposed to a potential cave-ins from the unsupported walls of the excavation and the spoils pile which was not at least 2-feet back from the edge of the excavation.
