29 CFR 1926.405(a)(2)(ii)(I):Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. On or about November 7, 2023, an extension cords passing through a doorway was not protected from damage, exposing employees to a possible electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 7, 2023, an employee walking/ working from an elevated balcony was not protected from a fall, exposing the employee to a fall of 14.5 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about November 7, 2023, an employee working at heights exceeding six feet was not provided training to recognize and minimize fall hazards during construction activities, exposing the employee to a fall of 14.5 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
