29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  Carwash Exit Side: On or about November 6, 2023, the employer exposed employees to struck-by hazards in that hardhats were not worn while installing glass window frames overhead.

29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) Carwash Exit Side: On or about November 6, 2023, the employer exposed an employee to a fall hazard in that, a harness with a lanyard attached to the boom or basket was not worn by each employee that worked from an aerial lift.

29 CFR 1926.95(d)(1): Except as provided by paragraphs (d)(2) through (d)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, shall be provided by the employer at no cost to employees:  (a)  JR General Glass Waterproofing LLC: On or about November 6, 2023, the employer exposed employees to struck-by and fall hazards, in that personal protective equipment (PPE), including protective helmets and fall protection, was not provided to employees that at no cost to employees.
