29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a)	In the warehouse: On or about November 08, 2023, employees were required to operate a Hyster forklift (S/N J004V04616S) that was outfitted with a paper roll clamp attachment that did not have written approval from the manufacturer of the forklift to be used on the forklift.  b)	In the warehouse: On or about November 08, 2023, employees were required to operate a Toyota forklift (S/N C3252) that was outfitted with a carton clamp attachment that did not have written approval from the manufacturer of the forklift to be used on the forklift.  c)	In the warehouse: On or about November 08, 2023, employees were required to operate a Yale forklift (S/N GLC100MGNGAE085) that was outfitted with a carpet roll attachment that did not have written approval from the manufacturer of the forklift to be used on the forklift.

29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a)	In the warehouse: On or about November 08, 2023, employees were required to operate a Hyster forklift (S/N J004V04616S) that was outfitted with a paper roll clamp attachment without having requested the data plate updated with information regarding the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  b)	In the warehouse: On or about November 08, 2023, employees were required to operate a Toyota forklift (S/N C3252) that was outfitted with a carton clamp attachment without having requested the data plate updated with information regarding the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  c)	In the warehouse: On or about November 08, 2023, employees were required to operate a Yale forklift (S/N GLC100MGNGAE085) that was outfitted with a carpet roll attachment without having requested the data plate updated with information regarding the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.

29 CFR  1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading:  a) Loading docks: On or about November 08, 2023, and at times prior thereto, employees were required to use forklifts to load trailers that were not chocked to prevent their movement
