29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) At 44 Sea Garden St, Alys Beach FL: On November 8, 2023 and at times prior thereto; the employer allowed his employee to perform framing activities from a scaffold without fall protection exposing his employee to a fall hazard of 30 feet to the ground below.

29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.   a) At 44 Sea Garden St, Alys Beach FL: On November 8, 2023 and at times prior thereto; the employer allowed his employees to perform masonry activities without fall protection training exposing his employees to a fall hazard of approximately 30 feet to the ground below.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) and as referenced by the note contained in 1926.602(d):   a) At 44 Sea Garden St, Alys Beach FL: On November 8, 2023 and at times prior thereto; the employer allowed his employee to operate a forklift without completing a forklift training program that met the requirements of OSHA standard 1910.178(l) powered industrial trucks exposing employees to crushing and struck-by injuries.
