An employee was performing concrete work on top of a formwork wall structure approximately 9 feet above the ground and was not protected from falling. Note: Abatement documentation is required. Pursuant to 29 CFR 1903.19.

Formwork in the basement was not designed to withstand the concrete load. The formwork collapsed. Note: Abatement documentation is required. Pursuant to 29 CFR 1903.19.
