At 76 Otter Slide Rd, Eastpoint, FL, on or about October 30, 2023, Carlisle Architectural Metals LLC, exposed employees to crushing injuries in that lockout/tagout procedures were not utilized while performing service and maintenance to a Schechtl metal bending/forming machine.

29 CFR  1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:  a.  At the fabrication shop in 76 Otter Slide Road, Eastpoint, Florida, on or about October 30, 2023, the outside employer, Carlisle Architectural Metals LLC, did not ensure their personnel coordinated with the onsite employer, Standing Seam Roofing LLC, about each other's energy control procedures, exposing employees to caught-in between and crushing hazards.
