At 7501 N Florida Ave., Tampa, FL:  On or about November 14, 2023, the employer exposed employees to crush by hazards, in that,  the Owner was operating  a JLG forklift without training certification.

At 7501 N Florida Ave., Tampa, FL:  On or about November 14, 2023, the employer exposed employees to crush by hazards, in that,  the Owner attached  a hand made wood box to the JLG without manufacturer written approval, such modification, change  the capacity and operation of the JLG forklift.

At  7501 N Florida Ave., Tampa, FL:  On or about November 14, 2023 he employer exposed employees to fall  hazards, in that, employees were performing roofing work during which time the employer failed to provide any safety related training record.
