29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 11/14/2023, in the Cutting department; Lockout/Tagout (LOTO) procedures were not utilized when employees performed servicing or maintenance such as, but not limited to: setting up the dies of Bobst Die cutter 1260 and SP 104E, exposing them to the unexpected start of the machine. b) On or about 11/14/2023, in the Production Floor; Lockout/Tagout (LOTO) procedures were not utilized when employees performed servicing or maintenance such as, but not limited to: cleaning, and setting up the dies of the Web feed die press- Zerand, exposing them to the unexpected start of the press. c) On or about 11/14/2023, in the Production Floor; Lockout/Tagout (LOTO) procedures were not utilized when employees performed servicing or maintenance such as, but not limited to: unjamming, and setting up the Right Angle machine, 6 FX machine, and Straight Line machine, exposing them to the unexpected start of the machines. ABATEMENT DOCUMENTATION REQUIRED
