On or about November 13, 2023, and at times prior to, at a soil drilling site, the rotating drill was not stopped prior to an employee exiting the drilling control platform and was exposed to contact with the rotating drilling auger.

29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. On or about November 13, 2023, at a soil drilling site, there was no guardrail, travel restraint system, cover, or guard for the rotating auger when employees stepped off the drilling control platform and were exposed to trips and falls to the rotating auger.
