29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) Lumber Yard - On or about 11/16/2023, and at times prior, employees were exposed to struck-by hazards in that, the employer failed to ensure that evaluation of powered industrial truck operator's performance were provided every three years to employees operating Hyster forklift (Model #170) at the facility.
