29 CFR 1926.1203(a): Before it begins work at a worksite, each employer must ensure that a competent person identifies all confined spaces in which one or more of the employees it directs may work, and identifies each space that is a permit space, through consideration and evaluation of the elements of that space, including testing as necessary. a) On November 16, 2023, at the High-Volume Tempering Area, the employer exposed employees to electrical shock and entrapment hazards, in that the employer failed to ensure all confined spaces were identified by a competent person prior to employees entering the confined space.

29 CFR 1926.1205(a): Before entry is authorized, each entry employer must document the completion of measures required by �1926.1204(c) by preparing an entry permit. a) High Tempering Volume Area located at 1595 Heraeus Blvd, Buford, GA: On or about November 16, 2023, the employer did not document the completion of measures required by paragraph 1926.1204(c) of this standard by preparing an entry permit before the entrant entered the pit exposing employee to crushing hazards.
