29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)	On or about November 14, 2023, employees doing masonry work of applying bricks to the outside walls of a new build commercial Sonic Restaurant and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:       (a)	On or about November 14, 2023, employees doing masonry work of applying bricks to the outside walls of a new build commercial Sonic Restaurant and were exposed to eye injuries by not wearing safety glasses while using a Makita hand grinder, model no. XAG03.

29 CFR  1926.453(a)(2):aerial lifts may be "field modified" for uses other than those intended by the manufacturer provided the modification has been certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification:  (a)	On or about November 14, 2023, employees doing masonry work of applying bricks to the outside walls of a new build commercial Sonic Restaurant while using a JLG Skytrak telehandler lift where a 48X124 wood man-made pallet platform was attached to the forks. The employees were exposed to fall hazards.

Employees were exposed to struck by hazards.
