29 CFR 1910.212(a)(3)(ii):  Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) 148 West Airport Road, Lititz, PA 17543 - an employee was operating a Header National 1/2-inch bolt maker machine (Model LSDS, Serial #: 34026) with the top lid / doors that provide guarding to the point of operation open, which did not restrict employee access to the point of operation during the operating cycle, on or about November 17, 2023.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, rotating parts, and flying chips:   a) 148 West Airport Road, Lititz, PA 17543 - an employee was operating a Header National 1/2-inch bolt maker machine (Model LSDS, Serial #: 34026) with moving parts at the lower portions of the machine within 12-inches of the operator's position, and the machine was not equipped with guarding at the lower portions of the machine, on or about November 17, 2023.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a) 148 West Airport Road, Lititz, PA 17543, Davenport and Boltmaker locations - employees were exposed to slip and fall hazards, as there was an oily substance on the walking / working surfaces, on or about November 17, 2023.  Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
