29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.   a) On or about November 17, 2023, at building No. 4403, employees were exposed to 35'- 4" fall hazards while installing metal shingles on the 5/12 pitched roof, in that; a roof anchor (Safe-Tie Bucket Model 00815-QC) was not installed in accordance with the manufacturer's instructions, thus incapable of supporting 5000 lbs.

29 CFR  1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a) On or about November 17, 2023, at building No. 4403, employees were exposed to 35'-4" fall hazards while installing metal shingles on a 5:12 sloped roof, in that the employer did not remove from service fall protection ropes that were damaged and showed signs of being frayed and cut.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about November 17, 2023, at building No. 4403: the employer did not prepare and maintain a written fall protection training program certification for employees exposed to fall hazards of 35'-4" while installing metal shingles.
