29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about November 21, 2023, and at times prior thereto, an employee was framing a one-story residential house under construction while working above multiple other employees who did not have head protection, exposing those employees to the hazard of being struck by falling tools, materials and debris from a higher level.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  On or about November 21, 2023, and times prior thereto, framers were using pneumatic nail guns and hand-held circular saws without wearing eye or face protection, exposing the framers to the hazard of being struck in the eye(s) by foreign objects or flying projectiles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about November 21, 2023, and times prior thereto, a framer was working on the top plate of a one-story residential house under construction without the use of fall protection, exposing the framer to the hazard of falling over 6-feet to a lower level.
