Located at 1464-1474 Lucky Cross Run, building 7, Reynoldsburg, Ohio: On or about November 20, 2023, the employer failed to train and instruct employees on the recognition of hazards associated with fall hazards and struck by hazards while employees performed residential roofing with nail guns from heights of up to 17.5 feet from the ground.
