29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Envelope Line 1, Front Feed: On or about November 20, 2023, employees are exposed to crush-by hazards while operating machinery due to an unguarded ingoing nip point. b) Envelope Line 1, Rear Feed: On or about November 20, 2023, employees are exposed to crush-by hazards while operating machinery due to an unguarded ingoing nip point. c) Envelope Line 2, Front Feed: On or about November 20, 2023, employees are exposed to crush-by hazards while operating machinery due to an unguarded ingoing nip point. d) Envelope Line 2, Rear Feed: On or about November 20, 2023, employees are exposed to crush-by hazards while operating machinery due to an unguarded ingoing nip point. e) Envelope Line 1, unwinder: On or about November 20, 2023, employees are exposed to crush-by hazards while feeding plastic film into the machine, due to an unguarded ingoing nip point. f) Envelope Line 2, unwinder: On or about November 20, 2023, employees are exposed to crush-by hazards while feeding plastic film into the machine, due to an unguarded ingoing nip point. 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Envelope line 1, welding roller: On or about September 20, 2023, employees were exposed to crush-by and burn hazards while operating the machine due to unguarded, heated rollers that fuse together different materials prior to folding the envelope, which resulted in an employee injury. b) Envelope Line 2, welding roller: On or about November 20, 2023, employees were exposed to crush-by and burn hazards while operating the machine due to unguarded, heated rollers that fuse together different materials prior to folding the envelope. c) Envelope Line 1, welding/needle bars: On or about November 20, 2023, employees were exposed to crush-by and burn hazards while operating the machine due to unguarded, needle and heated welding bars. d) Envelope Line 2, welding/needle bars: On or about November 20, 2023, employees were exposed to crush-by and burn hazards while operating the machine due to unguarded, needle and heated welding bars.
