29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a.  On or about September 28, 2023, in the Laser Department, the written control program did not include machine specific step by step procedures to control and lockout energy sources, or provide alternative protective measures, for employees who may perform service and/or maintenance activities, to include setup and clearing jams, on the A516 laser printing press while equipment was energized.  A press operator sustained a right middle fingertip amputation while attempting to unjam sticker labels from printing press without equipment being deenergized and implementing lockout/tagout procedures.  Employees were thereby exposed to machine hazards associated with ingoing nip points and rotating parts.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  a.  On or about September 28, 2023, in the laser department, the employer failed to provide adequate training for employees performing service and/or maintenance activities, to include setup and clearing of jams, to ensure the knowledge and skills for the safe application, usage and removal of energy control devices were acquired.  A press operator sustained an amputation to the right middle fingertip while attempting to clear a jam of sticker labels on the A516 laser printing press without machine being de-energized and locked out/tagged out.
