At the construction site located at South 14th Ave. & Mike Miller Pkwy. Dodge City, KS, employee(s) engaged in trenching and excavation operations, installing water and sewer lines, were exposed to struck-by and collapse hazards associated with trench work. Employee(s) were working inside a trench approximately 5 feet 6 inches (5 ft. 6 in.) in depth without the use of an adequate protective system.

Dodge City, KS, employee(s) engaged in trenching and excavation operations, installing water and sewer lines, were exposed to fall hazards of approximately 5 feet 6 inches from the edge of the trench to the bottom in that the 6-foot Werner A-frame ladders used to access/egress the trench were not extended at least 3 feet above the upper landing of the trench.
