29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about November 15, 2023, and at times prior thereto, an employee was cutting tree branches and was exposed to eye or face hazards from flying particles and debris. The employer did not ensure employees used eye protection and this condition exposed employees to eye injuries.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about November 15, 2023, and at times prior thereto, an employee was cutting tree branches and was exposed to head injuries from falling objects. The employer did not ensure employees used protective head equipment which exposed employees to head injuries, cuts and bruises, and struck-by hazards.
