29 CFR  1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:   a) Back Storage Area: On or about 11/29/2023, employees were not trained on fire extinguisher use and incipient stage fire fighting including, but not limited to, fire extinguisher types, when to use an extinguisher, and hazards associated with incipient fires.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a) Facility: On or about 11/29/2023, the employer did not provide effective training on Hazard Communication to employees at the facility who used chemicals including, but not limited to, Alpet Surfaced Sanitizer Wipes, J-512 Sanitizer (a corrosive), Crew Restroom Floor & Surface SC Non-Acid Cleaner (a corrosive), and Cirtuswirx2.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
