29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a)Engineered Concrete Products, LLC at 22466 Bennett Road, Rapid City, SD 57702: On or about at times prior to November 20, 2023, the employer did not ensure the use of a fall protection system while employees were working on the core and jacket performing concrete pouring operations. This condition exposes employees to fall hazards that were greater than four feet.

The hook was observed with the safety latch missing. This condition exposes employees to struck-by hazards.

This condition exposes employees to struck-by hazards.  (b)Engineered Concrete Products, LLC at 22466 Bennett Road, Rapid City, SD 57702: On or about at times prior to November 29, 2023,  the employer did not ensure that safety guards were used on the Masterforce Variable Speed portable grinder prior to employees conducting grinding operations. This condition exposes employees to struck-by hazards.
