29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about November 30, 2023, and dates prior, the employer did not develop, document and utilize  specific energy control procedures for, but not limited to the Sinvo automated assembly Machine Cells (Line 1 and Line 2 - Wave 3 Pack Out),  that outline the steps for shutting  down, blocking, and isolating mechanical and other energy sources.  This exposed employees to crushing and struck-by hazards.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   (a) On or about November 30, 2023, and days prior, authorized employee training was not provided to all  maintenance Technicians and other authorized personnel in the recognition of hazardous energy sources, and practices and methods required to safely deenergize machines/equipment, including but not limited to fluid delivery systems, HVAC systems, air compressor systems, and manufacturing machines and systems.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) On or about November 20, 2023,  at the Sinvo automated assembly machine cell (Line 1 - Wave 3 Pack Out), the employer by-passed the 22  integrated access door interlocks allowing employee(s) to access multiple point of operations to clear jams and to load material, to include but not limited to the energized conveyor, machine part presses and adhesive stations. This exposed employees to struck-by, caught-in and crushing hazards.  (b) On or about November 30, 2023, and days prior, at the Sinvo automated assembly machine cell (Line 2 - Wave 3 Pack Out), the employer by-passed the 22 integrated  access door interlocks allowing employee(s) to access multiple point of operations to clear jams and to load material, to include but not limited to the energized conveyor, machine part presses and adhesive stations.
