29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The program was not updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer did not include in the program the following provisions of this section, as applicable:  a) On or about December 01, 2023, and at times prior the employer did not develop a written respiratory protection program with required work-site specific procedures and elements for required respirator use when requiring employees to wear tight-fitting respirators and N95 while hand scraping concrete forms exposing employees to silica dust.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a) On or before December 01, 2023, and at times prior, the employees were exposed to lacerations while using hand tools, overhead hazards from overhead hoists, foot crush hazards from moving equipment and the employer did not assess the hazards employees in precast were exposed to and verify through a written certification that the required workplace hazard assessment had been performed.

29 CFR 1910.1200(e)(1): Employer did not develop or implement a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  a) On or about December 01, 2023, and at times prior, the employer did not develop or implement a written hazard communication program when employees were exposed hazardous chemicals such as, but not limited to, silica from concrete, oil used to lubricate the forms, welding fumes and welding consumables.
