29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On December 4, 2023, at the above addressed jobsite, employees were exposed to a struck-by hazard from falling tools, materials, the bucket and arm of an excavator and debris when working around and in a trench below employees and equipment.  The employees were not wearing a hard hats.  No abatement certification or documentation is required for this item.
