29 CFR  1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    a) Eastern Propak LLC.- Facility located at 800 Ellis St. GLASSBORO, NJ 08028 : On or about  09/18/2023 the employer did not record a work related injury of an employee that suffered a left hip/flank contusion injury and was out of work for approximately 3 days, on the 300 OSHA 300 log or its equivalent.  b) Eastern Propak LLC.- Facility located at 800 Ellis St. GLASSBORO, NJ 08028 : On or about 11/02/2023 the employer did not record a work related injury of an employee that suffered a right index finger laceration and was out of work for approximately 4 days on the 300 OSHA 300 log or its equivalent.
