29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  88 Dart Street, Hartford, Connecticut roofing project: On November 29, 2023, an employee was exposed to a fall hazard of approximately 20 feet while performing residential roof repair and installation of asphalt roofing shingles as the employee was not using fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  88 Dart Street, Hartford, Connecticut roofing project: On November 29, 2023, employees were using an extension ladder to access the roof as the siderails of the ladder used to access the roof only extended approximately a foot and a half above the upper landing surface exposing them to a fall hazard of approximately 20 feet.
