29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about December 6th, 2023, and at times prior, the employer, did not ensure that employees conducting re-roofing operations on a single-family residential home were protected from falls greater than 6 feet.

29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that would not deflect:  (a) On or about December 6th, 2023, and at times prior,  the employer, did not ensure that employees accessing the upper landing surface of a single-family residential home were protected from falls greater than 6 feet by ensuring that a  portable ladder that would extend at least 3 feet above the landing surface or was secured was utlized.
