29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that would not deflect: (a) On or about December 6th, 2023, and at times prior, the employer, did not ensure that employees accessing the upper landing surface of a single-family residential home were protected from falls greater than 6 feet by ensuring that a portable ladder that would extend at least 3 feet above the landing surface or was secured was utlized.
